An insulin resistance test gives you a focused view of how your body handles blood sugar. It measures your fasting glucose and fasting insulin and uses them to calculate your HOMA-IR, a widely used score for mapping insulin resistance.
You order the test online and have your blood drawn at a certified sample point. You need no referral and receive your results within a few working days, including a doctor's assessment, so you gain preventive insight into your metabolic health.
Why this test?
Insulin resistance often develops quietly. Your fasting insulin can rise to keep your blood sugar normal long before your glucose value itself changes, so an insulin resistance test may surface metabolic change earlier than a standalone glucose measurement.
Testing gives you a snapshot of your metabolic health. It is not a diagnosis, but a data point you can discuss with a doctor alongside your symptoms and any other values. For busy professionals, it can be an efficient way to get an early handle on signals such as ongoing fatigue or weight gain around the middle.
Who is this test for?
This test may suit people who:
- want to map their metabolic health preventively
- experience a higher risk of insulin resistance, for example through excess weight or family history
- have symptoms that might relate to blood sugar regulation, such as fatigue after eating
- want to follow a previous value over time
Would you like a broader view of your metabolism? A doctor may advise adding other markers alongside these values.
What is tested?
This panel measures two biomarkers in your blood and calculates a third value from them:
- Glucose (fasting): your fasting blood sugar.
- Insulin (fasting): the hormone that moves glucose into your cells.
- HOMA-IR: the calculated insulin-resistance score from the two values above.
HOMA-IR is not measured separately but derived from your fasting glucose and insulin using the formula (glucose × insulin) / 22.5. You can read more on the pages about fasting glucose and fasting insulin.
What can this test tell you?
Your HOMA-IR score can say something about how sensitive your body is to insulin right now. A higher score may point towards more insulin resistance; a lower score may fit better insulin sensitivity.
Because an insulin test can vary from one moment to another, a single result is always a snapshot. A doctor therefore looks at the wider context: your glucose, your insulin, your HOMA-IR and your symptoms together.
How is the sample collected?
For this test, blood is drawn at a certified sample point. There are 650+ locations across the Netherlands, which you choose yourself after ordering, at a time that suits you.
You do not need to see your GP first: this is a blood test without a referral. The draw is done by trained staff and usually takes only a few minutes.
When is this test useful?
A HOMA-IR test can be useful if you want to know preventively how your blood sugar regulation stands, or if you have symptoms you cannot quite place, such as energy dips or weight gain around the middle.
If you had an abnormal glucose or insulin test before, tracking your HOMA-IR can help you see how it develops. You always interpret the result together with a doctor.
What do the results mean?
Your result shows your fasting glucose and insulin with their reference ranges, plus your HOMA-IR with a short explanation. The HOMA index is a ratio: what counts as healthy varies by lab, age and timing, so a number on its own says little.
A higher HOMA-IR may point to more insulin resistance, but never stands alone. Only a doctor can properly interpret your result within your personal situation.
What happens after the results?
Your results are available online within a few working days. You see your glucose, your insulin and your HOMA-IR with their reference ranges and a clear explanation.
Every blood test result includes a professional assessment from a BIG-registered doctor. For treatment decisions, discuss your results with your GP.
Frequently Asked Questions
Yes. Both your fasting glucose and fasting insulin need an empty stomach, so fast for 8 to 12 hours before the draw. Water is fine. A non-fasting sample makes your HOMA-IR score unreliable.
No. The lab measures your fasting glucose and fasting insulin; your HOMA-IR is calculated from those two values. That is why the test reports all three.
No, you do not need a referral for this test. You can share your results with your GP afterwards.

Preparation
-
Pause biotin
RequiredDo you take a supplement with biotin (vitamin B8, common in hair, skin and nail supplements)? Stop it 72 hours before the draw. Biotin interferes with the measurement of these values.
-
Come fasting
RequiredFrom 22:00 the evening before, eat and drink nothing except water. That is at least 8 hours. Skip coffee, tea, chewing gum and smoking that morning. Plan the draw before 10:00.
- Water is always fine. Drink as usual beforehand, it makes the draw easier.
- Take your usual medication, unless your own doctor told you otherwise.
