Liver detox: do cleanses work, or is it a myth?

The January detox is back in every web shop: tea, capsules and a ten-day plan that supposedly "resets" your liver. Here is my plain view after reading the claims: you are paying for something your liver already does, for free, around the clock. You do not need a cleanse to detox, you need a liver that is not needlessly overloaded.

Below I separate the marketing from what actually improves your liver values, with a table showing what the evidence says about each detox promise.

Can you detox your liver?

Your liver is your natural detox organ. It breaks down alcohol, medicines and waste products continuously, with no help from a cleanse. A healthy liver needs no special cleaning. What helps most is less load: less alcohol, less sugar and a healthy weight. The Dutch Nutrition Centre (Voedingscentrum) is clear on this: "detox" is not a recognised medical term, and healthy eating does more than any cleansing product.

Do detox cleanses and liver tea work?

For detox diets and cleansing teas there is little to no clinical evidence that they remove toxins or "clean" the liver. Most studies are small and methodologically weak. If you feel better after a cleanse, it is usually because you temporarily cut alcohol, sugar and processed food, not because of the cleanse itself. You can get that effect without expensive capsules.

Are liver supplements like milk thistle useful?

For popular "liver supplements" such as milk thistle the evidence is weak and inconsistent. More importantly: some herbal extracts in high doses can actually burden the liver. "Natural" does not automatically mean safe. The Dutch public-health institute (RIVM) tracks reports of liver damage after herbal preparations, with high-dose green tea extract named repeatedly. When in doubt, discuss supplements with your doctor or pharmacist.

What actually improves your liver values?

The approach that works consistently in research is about lifestyle:

  • Losing weight if overweight: even 5 to 10% weight loss improves fatty liver.
  • Less alcohol: mainly lowers a raised gamma-GT. The Dutch Health Council (Gezondheidsraad) advises no alcohol, or at most one glass a day.
  • Less sugar and refined carbs: fatty liver is tied to metabolism.
  • Regular exercise: improves your liver even without weight loss.

So why does a cleanse feel like it works?

Many people feel lighter and more energetic after a detox week and conclude the cleanse must work. What usually happens: during that week you often drop alcohol, soft drinks, snacks and late meals, and drink more water. Those are exactly the behaviour changes that move your liver values in a good direction. The effect comes from what you leave out, not from the tea or capsules you add. The good news is that you can keep that gain without ever buying a cleanse again, simply by keeping those habits.

Watch the downside too. Some detox products are strongly diuretic or laxative. You then lose mainly water, not fat and not "toxins". With prolonged use that can disrupt your fluid balance and salts. "Cleansing" is not always harmless.

What does a liver panel actually measure?

Instead of guessing whether your liver is "full", you can simply measure it. A liver panel looks at enzymes that leak from your liver cells on irritation or damage, such as ALAT and gamma-GT. A raised gamma-GT, for example, is a sensitive hint towards alcohol, while ALAT points more towards fatty liver. No cleanse lowers those numbers; less alcohol, less sugar and a healthy weight do.
