Your standard cholesterol test measures how much cholesterol your particles carry, not how many particles you have. That sounds like a detail, but it is exactly where two people with the same LDL can still have different risk. Markers like ApoB, hs-CRP, homocysteine and Lp(a) partly fill that gap. My take: for most people a plain cholesterol test is enough, but with a loaded family history or an unexpected profile, these markers can make the difference.
What exactly do these markers measure?

What is ApoB and what does it tell you?
ApoB counts the actual number of harmful cholesterol particles in your blood. Each LDL particle carries exactly one ApoB protein, so ApoB measures the number of particles, not just the cargo. In some people, especially with a normal LDL but many small particles, ApoB can reflect risk more accurately than LDL alone.
What is hs-CRP?
Hs-CRP is a sensitive measurement of CRP, an inflammation protein. A mildly raised hs-CRP can point to low-grade inflammation in your blood vessels, which can relate to your heart risk. Note: an acute infection or cold temporarily pushes CRP up, so hs-CRP is best measured when you are not ill.
What is homocysteine?
Homocysteine is an amino acid your body breaks down with the help of vitamin B12, B6 and folate. A raised value is sometimes linked to vascular damage, though that link is weaker than once thought. A raised homocysteine can also simply point to a shortage of these B vitamins.
What is Lp(a)?
Lp(a) is a largely hereditary variant of LDL that barely changes with lifestyle. A high Lp(a) can raise your risk, even if your ordinary cholesterol is fine. It is usually measured once. Read more in our article on Lp(a).
How do you read these markers together?
None of these markers stands alone. Their strength lies precisely in combination with your ordinary lipid profile. A few patterns doctors often recognise:
- Normal LDL, high ApoB: can point to many small, dense particles. Your particle count is then less favourable than your LDL suggests.
- Favourable cholesterol, high Lp(a): points to a hereditary component that your lifestyle does not reveal.
- Mildly raised hs-CRP without infection: can fit low-grade vascular inflammation, but is always assessed in context.
- High homocysteine: can simply reflect a shortage of B12, B6 or folate and is often influenced by diet or supplementation.
Precisely because the explanation differs per pattern, a professional assessment is more valuable than stand-alone numbers.
When are these markers worthwhile?
For many people a plain lipid profile gives enough to go on. These extra markers are mainly useful with cardiovascular disease in the family, an unexpected profile, or when your ordinary cholesterol and your clinical picture do not match up. The Dutch Heart Foundation (Hartstichting) stresses that such markers support the conversation with your doctor, but never replace the broader risk assessment.

What these markers do and do not tell you
A common misconception is that an advanced marker "recalculates" your heart risk. It does not. It adds an extra layer to an assessment your doctor makes based on your age, blood pressure, smoking, blood sugar and family history. A few nuances worth remembering:
- One deviating marker is not a diagnosis: it is a signal to look further, not a reason to panic.
- Timing matters: do not measure hs-CRP just after an infection, illness or intense training, because the value is temporarily raised.
- Not everything is equally solid: the link between homocysteine and heart risk, for example, is weaker than that of ApoB or Lp(a).
- Repeating is sometimes pointless: Lp(a) stays almost the same throughout your life, so one measurement usually suffices.
With these nuances you avoid giving a stand-alone number more weight than it deserves.
Frequently asked questions
Do these markers replace my ordinary cholesterol test?
No. They are an addition. Your ordinary lipid profile remains the foundation, and these markers refine the picture in case of doubt or a loaded family history.
Do I need to fast?
For ApoB and hs-CRP, fasting is usually not needed. For a full lipid profile with triglycerides, fasting is advisable. Follow the advice you receive with your test.
A deviating value can point to increased risk, but is not a diagnosis.
