Troponin T: the troponin T test for the heart explained

Troponin T is the most sensitive marker for detecting even minor heart muscle damage. Understanding this marker is key for comprehensive cardiac health awareness.

Deze afkapwaarden gelden voor de hoogsensitieve troponine T-test (hs-cTnT, Roche Elecsys). 14 ng/l is het 99e-percentiel van een gezonde populatie; Nederlandse laboratoria hanteren deze grens voor mannen en vrouwen gelijk. Zij duiden een waarde onder 14 ng/l als geen aanwijzing voor hartspierschade, 14 tot 100 ng/l als dubieus (twijfelachtig) en boven 100 ng/l als passend bij hartspierschade. Belangrijk: een normale troponinewaarde sluit een hartinfarct niet uit, en een verhoogde waarde bewijst er geen. Troponine is een test voor de acute diagnostiek bij klachten (zoals pijn op de borst), beoordeeld door een arts samen met het klachtenbeloop en een ECG - niet een screeningstest voor gezonde mensen, en nooit te beoordelen op een enkele waarde (bij een hartinfarct stijgt en daalt troponine over enkele uren; de diagnose vereist herhaalde metingen). Een verhoogde waarde kan ook passen bij nierfunctiestoornissen, hartfalen, longembolie, sepsis of na zware inspanning. Bij een verhoogde uitslag of bij acute klachten: neem direct contact op met een arts of bel 112. In de literatuur worden ook sekse-specifieke grenzen beschreven (vrouwen lager, mannen hoger); Nederlandse laboratoria rapporteren tegen de gezamenlijke grens van 14 ng/l.
