Always cold feet usually happen because your body sends heat to your core and serves your feet last. If they stay cold in a warm room, that's more interesting: your iron stores, your thyroid or your circulation may be playing a part. The difference lies mostly in whether it's new.
I think this is one of the most underrated complaints there is. Almost everyone knows someone with permanently cold feet, and almost nobody has ever had it looked into.
Why do your feet stay cold?
Your feet sit furthest from your heart and carry little muscle tissue of their own to make heat. In the cold, the small blood vessels there close down first so your organs stay warm. That's a healthy response, not a fault.
It only becomes a question when the response happens without a trigger. Cold feet on a summer evening on the sofa don't belong in that category.
Thuisarts, the Dutch GP information service, describes cold feet on their own as usually harmless. The surrounding context decides whether more might be going on.
When does it simply belong to your build?
Low muscle mass, low body weight and low blood pressure together mean less heat production and less push behind the blood flow. Someone slim who trains little therefore gets cold feet more often without anything being wrong. If it has been this way your whole life, that's usually the whole story.
There's no blood value that demonstrates this either. It's the answer left over when the rest shows nothing.
A simple check you can do yourself: do your feet warm up within twenty minutes under a blanket? Then your circulation is probably working fine.
Iron and thyroid: the two measurable suspects
When cold feet are new, these two sit at the top. Low iron stores reduce oxygen transport to your tissues, and a slower thyroid lowers your heat production. Both often bring fatigue alongside the cold, and both can be looked at with a single tube of blood.
With iron there's a catch. Your ferritin can already be low while your haemoglobin still reads normal, which makes a standard check look falsely reassuring. Clinical Medicine called iron deficiency without anaemia a diagnosis that matters in 2021.
For the thyroid it's TSH and free T4. A 2024 Lancet review describes the symptoms as broad and not very specific, which explains why people carry it for years.
An example of what that looks like. Two women of 35 with the same complaint: one has a ferritin of 68 µg/l, the other 11 µg/l, while both their haemoglobin readings sit comfortably normal. Same cold feet, a very different follow up conversation.
Reference limits differ per laboratory, so a number like that never stands on its own. Your GP weighs it together with your symptoms and the rest of your blood count.

Why your feet get cold before your hands
Your feet sit further from your heart, carry less heat producing muscle, and spend most of the day inside shoes that hold moisture. That moisture cools down and then pulls warmth away. So many people feel their feet as colder than their hands, without anything being wrong.
There's something else that gets forgotten: sitting. Sitting for long stretches slightly pinches the blood flow to your lower legs, especially with your legs crossed.
An example. You're at a desk from nine to five, your feet are cold by three, and after half an hour of walking they're warm again. That's a posture story, not a blood story.
If they stay cold after moving too, it gets more interesting. That's the point where the measurable causes become worth looking at.
I'd always run that walking test myself before having anything drawn. It costs half an hour and might save you a tube of blood.
When do cold feet need attention?
Call your GP if a foot suddenly turns cold, white or blue and hurts, if a small wound on your foot won't heal, or if you get calf pain while walking that disappears the moment you stand still. That last one is the signal most closely tied to your blood vessels.
Those are complaints someone wants to see, not measure. Don't wait for a test result first.

If it's new, and fatigue comes with it, your iron stores and your thyroid are the logical first step. You can have a ferritin test or a TSH test done without a referral and discuss the result with your GP afterwards.
