Vitamin D is mostly produced in the skin under the influence of sunlight. At Dutch latitudes that is barely possible between October and April, which makes a value like this ordinary in winter and spring. That does not make it unimportant: vitamin D contributes to bone formation, muscle strength and immune function.
Minerals
AttentionYour ferritin is 18 µg/L, just under the lower limit of 20. Ferritin is the storage form of iron, and that store depletes before hemoglobin falls.
That is exactly what we see here: your hemoglobin is still normal (8.4 mmol/L), but the reserve your body draws on is small. Fatigue is a well-recognised symptom of this combination, even without anaemia.
Cardiovascular

What It Measures
Cholesterol does not dissolve in blood, so it travels packaged inside lipoproteins: spheres with a fatty core and a protein coat. HDL stands for high-density lipoprotein, the smallest and densest type. The test does not measure those particles themselves, but the amount of cholesterol inside them. That distinction matters more than it looks: the result tells you something about the cargo, not about the number of particles and not about how well they do their job. HDL particles are the collection service of cholesterol transport. They take cholesterol up from cells around the body, including the foam cells in an early plaque in the artery wall, and deliver it to the liver, which disposes of it through the bile. This is called reverse cholesterol transport, and it is why a high HDL was seen as protective for decades. In the Netherlands the result is reported in mmol/l. Unlike LDL, the laboratory prints only a lower limit here, no upper limit, and that lower limit differs by sex: 1.0 mmol/l for men and 1.2 mmol/l for women. The difference is physiological. Oestrogen gives women a higher average HDL, so the bar sits higher for them. The practical consequence: a value of 1.1 mmol/l is normal in a man and too low in a woman. Do not simply compare your number with your partner's. One relationship you need to know in order to read the result: HDL moves inversely with triglycerides. A transfer protein swaps cholesterol out of the HDL particles in exchange for triglycerides from the fat-rich particles. The HDL particle ends up loaded with triglycerides, loses cholesterol and is cleared faster. Anyone with a lot of triglycerides in the blood is therefore left with a low HDL almost automatically. In practice a low HDL is rarely a stand-alone problem; it is usually the shadow of a raised triglyceride value and of the insulin resistance behind it. You do not need to fast for an HDL test. HDL barely changes after a meal, unlike triglycerides.

Start with the honest message: HDL is not a value you should try to push up directly. The trials in which HDL was raised substantially with drugs did not reduce cardiovascular events. An HDL number that goes up without anything changing in the underlying metabolism gains you nothing. So treat HDL as a yardstick for your metabolic health, and direct your effort at what genuinely improves that health. HDL rising along with it is a pleasant by-product, not a goal. What demonstrably helps: <ul><li><strong>Stopping smoking.</strong> Smoking lowers HDL, and the value recovers after quitting. It is also by far the measure with the largest gain for your cardiovascular risk.</li><li><strong>Less abdominal fat.</strong> Visceral fat and insulin resistance are the main reason HDL drops. Weight loss raises HDL modestly and lowers triglycerides at the same time.</li><li><strong>Endurance training.</strong> Regular aerobic exercise genuinely raises HDL, but modestly: meta-analyses put it at roughly 0.05 to 0.1 mmol/l, built up over months. Duration per session weighs more heavily than intensity.</li><li><strong>Fewer fast carbohydrates and less alcohol.</strong> Both lower your triglycerides, and because HDL moves inversely with triglycerides, your HDL comes up along with it.</li></ul> One thing not to do: drink alcohol in order to raise your HDL. Alcohol does raise the value, but that is a cosmetic effect without health gain, and the downsides of alcohol far outweigh it. If you use androgens or anabolic agents, that is by far the strongest influence on your HDL, and it is an unfavourable one. Discuss it openly with your doctor. And finally: never change anything about cholesterol-lowering medication on your own initiative on the basis of an HDL result. That medication targets LDL and ApoB, not HDL.
What It Measures
LDL stands for low-density lipoprotein, a particle that carries cholesterol through the blood. The LDL cholesterol result tells you how much cholesterol sits inside all of those particles together. It is a measure of cargo, not of particle number; that number is better approximated by non-HDL cholesterol and apolipoprotein B. What most people do not know is that LDL is rarely measured in a routine lipid panel. It is calculated. The classic Friedewald equation subtracts HDL from your total cholesterol and then subtracts the cholesterol sitting in the triglyceride-rich particles: LDL = total cholesterol minus HDL minus triglycerides divided by 2.2, with everything in mmol/l. That divisor of 2.2 is a fixed assumption about the ratio between triglycerides and VLDL cholesterol. The assumption is what makes a lipid panel cheap, and it is also the reason an LDL result can simply be wrong. The Martin-Hopkins method replaces the fixed divisor with a factor that varies with a person's own triglycerides and non-HDL. The Sampson-NIH equation holds up to roughly 9 mmol/l of triglycerides. There is also a direct LDL assay, which measures the cholesterol in LDL particles itself and therefore does not depend on triglycerides; direct assays are, however, less well standardised between manufacturers, so two laboratories can report different numbers for the same blood. So always know which kind of number you are holding. A calculated LDL is no more reliable than the three values it was built from.

Start with the cause, not with the diet. If your LDL has risen unexpectedly, have an underlying cause ruled out first, with the thyroid at the top of the list. Only then does a conversation about food make sense. The biggest effect on LDL comes from the type of fat in your diet, not the amount. Replacing saturated fat, meaning butter, hard cooking fats, fatty meat and full-fat dairy, with unsaturated fat from vegetable oils, nuts, seeds and oily fish measurably lowers LDL. Swapping saturated fat for fast carbohydrates instead does not produce that effect. Soluble fibre helps as well: oats and barley, pulses, fruit and vegetables bind bile acids in the gut, which makes the liver pull more cholesterol out of the blood. Plant sterols and stanols, added to some margarines and dairy products, also lower LDL; discuss with your doctor or pharmacist whether that makes sense in your situation. Exercise and weight loss mainly improve your triglycerides and your HDL and usually lower LDL only modestly. They remain worthwhile, because your risk is determined by more than one number. Stopping smoking does not lower LDL, but it lowers your cardiovascular risk faster than anything else you can do. One firm boundary to close on: never adjust lipid-lowering medication on your own, and do not stop it after a favourable self-ordered result. That result is often favourable precisely because of the medication.

1What It Measures
Haemoglobin (Hb) is the iron-containing protein in your red blood cells that binds oxygen. It picks up oxygen in your lungs, carries it around your body, and takes carbon dioxide back on the return trip. This test measures how much haemoglobin is present per litre of blood. In the Netherlands the result is usually reported in mmol/l; internationally g/dl is more common. The value is measured as standard within the complete blood count. Because almost all the oxygen in your blood is bound to haemoglobin, this value tells you directly how much oxygen your blood can carry. Your doctor will therefore almost always assess haemoglobin together with haematocrit, the red cell count and MCV.

Dietary iron is the foundation: red meat, legumes, wholegrain products and dark leafy greens. Vitamin C with a meal improves the absorption of plant-based iron, while coffee and tea inhibit it. Do not start iron supplements on your own initiative. Supplementing without a demonstrated deficiency is not useful and too much iron can be harmful; have your ferritin measured first. Drink enough before a blood draw: dehydration concentrates your blood and can artificially raise the result. A persistently low or high value should always be investigated by a doctor, because the underlying cause matters more than the number itself.
Inflammation

This test uses a highly sensitive assay to detect low concentrations of CRP that standard CRP tests cannot measure. It reflects subtle, chronic inflammation associated with vascular health.

Regular exercise, a Mediterranean-style diet, maintaining a healthy weight, and not smoking may help lower hs-CRP levels. Stress management and adequate sleep also contribute to reducing chronic inflammation.
Kidney
1What It Measures
Your eGFR estimates how many millilitres of blood your kidneys filter each minute. The e stands for estimated. The value is calculated rather than measured directly. The laboratory enters your creatinine, age and sex into an equation.<br><br>The standard equation is CKD-EPI. Its 2021 version deliberately works without a race coefficient. Older equations did apply one. They overestimated kidney function in Black patients and delayed referral. Some European laboratories also use the EKFC equation. CKD-EPI and EKFC can differ slightly in the same person. Check which equation your report names.<br><br>The figure is scaled to a standard body surface area of 1.73 square metres. It answers what your filtration would be at an average body size. For a very large or very small body the true filtration differs.<br><br>Creatinine comes from muscle tissue. Muscle mass therefore affects the result. Men on average carry more muscle than women. That is why sex appears in the equation. In a woman with little muscle the eGFR can look better than her kidney function truly is.

Stay well hydrated and maintain a balanced diet that is not excessively high in sodium or protein. Engage in regular physical activity, maintain a healthy weight, and avoid smoking. Liver
1What It Measures
The test measures how much ALT is circulating in your blood. The result is given in units per litre, abbreviated u/l. ALT stands for alanine aminotransferase.<br><br>Older reports sometimes carry the former name SGPT. It is the same enzyme and the same measurement.<br><br>The enzyme works inside the cell. It moves an amino group between two compounds. Your body needs that step to convert amino acids into fuel.<br><br>As long as a cell is intact, the enzyme stays inside it. Only when liver cells are damaged does ALT leak out.<br><br>Where ALT sits determines what the result is worth. The enzyme is concentrated mainly in the liver, far more than anywhere else. Small amounts sit in kidney, heart and skeletal muscle. That distribution makes ALT more liver-specific than AST, which occurs in many tissues at once.<br><br>This is why ALT rarely appears alone on a report. You usually see it beside AST, gamma-GT, alkaline phosphatase and bilirubin. Each enzyme points in a different direction.<br><br>ALT and AST come from the liver cell itself. Gamma-GT and alkaline phosphatase come from the bile ducts. That split guides the further investigation a doctor chooses.<br><br>The measurement itself says nothing about the cause. It only adds up how much enzyme is circulating. What a high ALT value means becomes clear only alongside the other liver values.

The biggest gain usually sits with weight, not with a supplement. Fatty liver is the most common reason for a mildly raised result in the Netherlands. Gradual weight loss combined with regular exercise is the best-supported approach.<br><br>A fall of roughly seven percent of body weight usually brings liver values down clearly. Do not expect an effect within a week.<br><br>Alcohol is the second dial you can turn. Heavy alcohol use burdens the liver directly. A period without alcohol often shows, on a repeat measurement, how much of the result was attributable to drink.<br><br>Sugar and fast carbohydrates deserve separate attention. Fructose from soft drinks and juices contributes to fat accumulation in the liver. That is a structural route, not a day-to-day one.<br><br>Go through your medicines and supplements with your GP or pharmacist. Include over-the-counter products and herbal preparations. Never stop a prescribed medicine on your own initiative because of a blood result. Discuss it first.<br><br>Finally, schedule the draw sensibly. Leave 48 to 72 hours after a heavy or unaccustomed training session.
Metabolic

Minerals
1What It Measures
Ferritin is a protein that stores iron. Each ferritin molecule can bind and safely store a large number of iron atoms, mainly in the liver, the spleen and the bone marrow. A small fraction of that ferritin leaks into the blood, and that fraction is what the test measures. Your ferritin level is therefore a reflection of how much iron your body has in storage. That makes ferritin different from the other iron values. Serum iron shows how much iron is circulating in your blood at the moment of the draw, a figure that swings considerably across the day and after a meal. Transferrin is the transport protein that carries iron through the blood, and transferrin saturation indicates what share of that transport capacity is actually occupied. Ferritin answers how much iron you hold in reserve, not how much is in transit. Results are reported in micrograms per litre (µg/l). The band below is the reference range of the laboratory that performs our tests. <table><thead><tr><th>Group</th><th>Reference value</th></tr></thead><tbody><tr><td>Men</td><td>30 – 400 µg/l</td></tr><tr><td>Women</td><td>15 – 150 µg/l</td></tr></tbody></table> The lower limit sits lower in women because the reference population it was derived from includes menstruating women, who lose iron structurally. It describes what is common in that group. It is not a statement that a smaller reserve is desirable or healthy in women. Two national bodies, the Dutch society for clinical chemistry and the hereditary haemochromatosis guideline, deliberately decline to publish a universal ferritin reference value. The reason is that the assay differs between laboratories, and the difference is not small: another platform uses 15 to 200 µg/l for men. Both therefore advise using the values of the performing laboratory. Always go by the reference values printed on your own report.
When Levels Are Low
An empty iron store often causes symptoms before anaemia develops. The most commonly reported are persistent fatigue that sleep does not resolve, breathlessness on exertion, dizziness, headache, cold hands and feet, pallor, brittle nails, hair loss and restless legs. Difficulty concentrating and reduced stamina are also described. Each of these complaints is non-specific and can have dozens of other causes, so on their own they prove nothing. They are, however, a good reason to have your iron status measured rather than wait. In men, and in women after the menopause, a low ferritin always requires an explanation, because menstrual blood loss falls away as a cause. Discuss this with your doctor.

How much iron you absorb from your diet depends more on the form than on the quantity. Haem iron from meat, fish and poultry is absorbed considerably better than non-haem iron from plant sources such as pulses, wholegrain products, nuts and dark green leafy vegetables. Anyone eating little or no meat therefore obtains iron mainly in the less absorbable form, and that is one reason stores can decline slowly without anything unusual being wrong. What you eat alongside it shifts absorption measurably. <strong>Vitamin C</strong> clearly increases the absorption of non-haem iron, so including a source of vitamin C with a plant-based meal is a simple and effective habit. The reverse also exists: the tannins in tea and coffee and the calcium in dairy inhibit absorption when taken with the same meal. Having tea or coffee an hour before or after eating rather than with it is a worthwhile difference for people with limited stores. Do not take iron supplements on your own initiative. Iron accumulates with untargeted intake, and supplementing without a demonstrated deficiency can be harmful. Whether supplementation is needed, in what form and for how long, is a decision for your doctor based on measured values. If your ferritin stays low despite adjustments, or is repeatedly raised, have the cause investigated rather than correcting it yourself.
Thyroid
1What It Measures
A TSH test measures how much thyroid-stimulating hormone circulates in your blood. TSH comes from the pituitary gland, a small gland at the base of the brain. The pituitary makes TSH and uses it to drive the thyroid. The thyroid then produces the hormones T4 and T3. The feedback between the two is log-linear, not proportional. A small fall in free T4 produces a much larger rise in TSH. That is what makes TSH the most sensitive single test for thyroid disease. It also explains something that often confuses people. TSH can sit far outside the reference range while free T4 is still normal. The direction is the opposite of what most people expect. If the thyroid runs slow, the pituitary pulls harder and TSH rises. If the thyroid makes too much hormone, the signal switches off and TSH falls. So a high value points to an underactive thyroid. No fasting is required. The unit appears on reports as mE/L, mU/L or mIU/L. Those are the same unit written three ways. TSH is rarely judged on its own. Free T4 almost always accompanies it, because only the pair completes the picture.

No diet or supplement reliably steers a TSH level. What you can influence is whether the measurement is accurate. Four points make most of the practical difference. Stop high-dose biotin well before the draw. Biotin is in many hair, skin and nail supplements, often at 5 to 10 milligrams. On many laboratory platforms that falsely lowers TSH and falsely raises free T4 and free T3. The result then mimics an overactive thyroid in a completely healthy one. Stopping 48 to 72 hours beforehand is the usual advice. Very high doses need longer. An ordinary multivitamin with a few micrograms does not cause this. Give blood at roughly the same time of day each time. Because of the daily rhythm that matters more than most people expect. If you take thyroid medication, give blood before that day's tablet. For TSH this is less critical than for free T4, but it keeps measurements comparable. Tell them which medicines you take. Lithium and amiodarone can disturb the thyroid. Proton pump inhibitors, calcium and iron reduce absorption of levothyroxine. That can explain an unexpectedly high value. Never adjust your own dose on the basis of a self-requested test.
Vitamins
2What It Measures
<p>This test measures <strong>25-hydroxyvitamin D</strong> (25-OH-D) in the blood. That is the storage form of vitamin D. It is also the only form that says anything about your reserve. The result is reported in nmol/l.</p><p>25-OH-D has a half-life of two to three weeks. The result therefore reflects supply over the preceding months, not yesterday.</p><p>The active form is called 1,25-dihydroxyvitamin D. It is deliberately <em>not</em> measured. It has a half-life of hours and is tightly regulated by parathyroid hormone (PTH).</p><p>When stores run low, the body increases the conversion. The active value can then look normal, or even raised, while the reserve is empty. Ordering the 1,25 form for a suspected deficiency is the classic clinical mistake.</p><p>The report should give total 25-OH-D, meaning D2 plus D3. That matters for anyone taking a plant-derived D2 product. Some immunoassays under-recover D2.</p>
When Levels Are Low
A low reserve causes no symptoms for a long time. When symptoms do appear, they are non-specific: <ul><li>muscle weakness in the thighs and hip girdle</li><li>difficulty rising from a chair or climbing stairs</li><li>bone and muscle pain</li><li>an unsteady gait</li></ul> Prolonged, deep deficiency produces osteomalacia in adults and rickets in children. Newly formed bone mineralises inadequately, causing bone pain and deformity. In older people a low value is associated with muscle weakness and a higher risk of falling. That drives fracture risk further up. These complaints fit dozens of other causes equally well. It is the number that prompts the conversation, not the symptom.

<p>The Netherlands sits at roughly 52 degrees north. From October through March the sun is too low here. Too little UVB (290 to 315 nm) reaches the skin. Through those months there is effectively no endogenous production, and the body draws on its summer reserve.</p><p>The month of the blood draw is therefore part of the result. A March value is not comparable with a September value.
